Unraveling the Magic of Enzyme Replacement Therapy

Today, we're diving deep into the world of modern medicine to uncover the wonders of enzyme replacement therapy, or ERT for short. If you've been part of our community at Pompe Warrior Foundation for a while, you've likely heard about ERT. But for the uninitiated, it might sound like something straight out of a sci-fi movie. 

So, what exactly is enzyme replacement therapy, and how does ERT affect those with Pompe disease? Let's break it down.


Enzymes: The Unsung Heroes of Our Bodies

Before we hop onto the ERT train, let's chat a bit about enzymes. These little powerhouses drive almost all processes in our bodies. Think of enzymes as workers on an assembly line, each with a specific job. When one worker is missing, the whole line can grind to a halt.

In the context of Pompe disease, the missing worker is an enzyme named acid alpha-glucosidase (GAA). GAA's job? Breaking down a sugar called glycogen. Without enough GAA, glycogen starts piling up, especially in muscle cells. 

Imagine your room when you skip cleaning for a month (or a year!). This buildup can lead to muscle weakness and even damage.

Enter ERT: The Backup Brigade


So, how does enzyme replacement therapy fit into all of this? ERT is like calling in backup workers to fill in for those missing or on a break. For our Pompe warriors, ERT provides the missing GAA enzyme. The specific enzyme used in ERT for Pompe disease is named alglucosidase alfa - a mouthful, I know, but hang tight! It's a replica of the human GAA enzyme.

Every few weeks, our warriors get this backup enzyme intravenously, which is just a fancy word for "into the vein." Once inside, the enzyme gets to work, helping to clear out the glycogen pile-up in cells.

The Effects of ERT on Pompe Disease

Here's where things get exciting. ERT isn't just a fancy science experiment. It's changing (and often saving) lives.

For the little warriors with infantile-onset Pompe disease, ERT can work wonders, especially for those kids who are diagnosed with newborn screening.  It can greatly boost heart function, clear up the heart's enlargement, and can help with lung functions.

For the older warriors with late-onset Pompe disease, ERT acts as a protective shield, either keeping muscle strength stable or sometimes improving it with additional therapy. Plus, it helps in better respiratory function, meaning fewer breathless moments.

The Downside (Because Science Isn’t Always Perfect)

Now, I'd love to tell you that ERT is the magic potion that cures all, but that wouldn’t be the full picture. While ERT is an absolute game-changer, it isn't a complete cure. ERT is shown in various studies to be 90% effective with the heart but at other times 30% effective in clearance.  ERT has been proven to be less effective in the larger muscle groups.  It is even less effective in the fast twitch muscle groups like the oral muscles.  And it cannot pass the blood-brain barrier..meaning any glycogen stored in the brain is NEVER touched by ERT.  Regular infusions are the name of the game, often for life, but never the ONLY thing you can do to help slow down the disease. 

And, as with all things medical, there's a small print. Some folks might experience allergic reactions. Others might see a reduced effectiveness over time if their body starts thinking of the infused enzyme as an uninvited guest. Or you continue to store and make more glycogen every week.   

The Silver Lining

Despite the challenges, some things are undeniable: The earlier a Pompe warrior gets diagnosed and starts ERT, the brighter the outlook. Early action is crucial, especially for our infantile-onset warriors. It can turn the tide, preventing irreversible damage and paving the path for a life full of possibilities. ERT, while being a game changer, does have limitations every patient should know and understand.  If you continue to make and store more glycogen by what you eat, you will continue to create a remainder of glycogen that the ERT cannot break down.  Understanding the truth in what ERT can and cannot do can help you make the appropriate adjustments to help the medicine be the most effective.

Enzyme replacement therapy has revolutionized how we approach and treat Pompe disease.
